The service is available to patients of any age in the London Boroughs of Barnet, Enfield and Haringey who have advanced, progressive disease. Any diagnosis is considered.

The multi-professional team works with primary care teams and other professionals in caring for a patient, their family and friends at home.

The team offers symptom control, counselling and other advice. A limited supply of equipment is available for home loan.

The team comprises:

• Clinical nurse specialists
• Social workers
• Doctors
• Physiotherapists
• Volunteer complementary therapists
• Volunteer "sitters"
• A consultation visit may be made to a patient at the request, but please note that no patient is visited without permission from their GP.

The full service is operational Monday to Friday, 9 to 5 with access to specialist advice and support available at all other times.
